Identify Unreported Patient Harm with AI

OIG has found that 69% of patient harm events are not captured in voluntary incident reporting systems. IHI estimates that number to be as high as 90%. HarmScout gives your organization the opportunity to improve patient safety by capturing 100% of patient harm events.

The first step in achieving Zero Preventable Harm is being able to identify preventable harms.

The Literature

These are the finding of two landmark studies that have guided our solution: 25% of patients suffer harm during the course of their hospital stay. Of those that suffered harm, 28-40% are deemed preventable harms. Said differently, 7-10% of all patients experience preventable harm during the course of their hospital stay. 69-90% of patient harms are not entered into an organization's incident reporting system due to the harm event not being considered to be harm or capturing that type of harm isn't standard practice at the organization.

HarmScout Solution

HarmScout leverages artificial intelligence and a clinical rules engine to monitor electronic health records and identify unreported patient harm. The platform automates approximately 90% of the monitoring and analysis workload in the background, enabling clinicians to focus their expertise on reviewing the remaining high-value cases.

Step 1

Automated Medical
Record Monitoring

HarmScout provides continuous surveillance of patient records across all categories of harm. Tasks that would otherwise require thousands of clinician hours to complete manually are automated by HarmScout in the background.

Step 2

Potential Harm Events Identified

HarmScout detects potential harm events that are not otherwise reported, which occur in an estimated 25% of patient visits. The categories of harm monitored can be tailored to the specific needs and priorities of each hospital.

Step 3

Validation of HarmScout Findings

Cases identified by HarmScout are forwarded to the designated clinicians for confirmation. Confirmed cases are forwarded to the adverse event management system for cause analysis and corrective action.
